The Wound Care Team scope of practice involves the attention of team members who are equipped with the skills to assess, monitor, prevent, and treat a wide variety of acute and chronic wounds such as pressure injuries, trauma-related wounds, burns, wounds and skin conditions related to vascular compromise, ostomies, and/or continence conditions, and diabetic foot conditions. Wound care nurses also help educate and provide care instructions for residents, families and caregivers in order for the condition to properly heal.
The Wound Care Team’s proactive evidence-based approach to the prevention and management of wounds and skin issues includes placing residents on appropriate low air-loss mattresses or specialized beds, performing full body skin assessments at regular intervals, integrating the services of in-house Registered Dietician to ensure adequate nutrition, weekly rounding with expert wound physician, employing simple to complex wound dressing changes, and utilizing therapeutic vacuum dressing devices as indicated.
