Individualized Plan
Plaza Healthcare admits the most complex patients of any skilled nursing facility in the southwestern United States so the therapists are well trained and extremely competent in the evaluation and treatment of a wide range of diagnoses and illnesses.
Due to this complexity, each therapist performs an extensive evaluation collaborating with the patient, their family, the clinical team, and the physicians to better understand the client specific goals to maximize the person’s recovery and overall quality of life.
The treatment plan will be designed with the patient’s current condition in mind but will change and evolve with the recovery process to maintain progression and enhance independence.
Each plan of care will be tailored to the patient’s schedule. For example, those patient’s receiving dialysis are often too tired after an exchange to benefit from therapy; so, the therapist will work with the Renal team to either treat before dialysis, or increase treatment times on non-dialysis days. Due to the complexity of medical issues the patient may experience during recovery, therapy demonstrates flexibility in the daily treatment plan, basing it on the patient’s current medical condition. Therapy may often focus on less rigorous treatments until the patient has regained the strength and ability to fully participate.
It is this flexibility and multidisciplinary approach which can maximize a patient’s ability to recover as quickly as possible. In addition, family are encouraged to be an active participant in their loved ones treatment plan and are welcome to attend any of the sessions.
Restorative Nursing
Every patient progress at their own pace and is some cases, hit plateaus. It is during these times of plateauing progression that Plaza Healthcare’s Restorative Nursing Program is most vital.
The Restorative Nursing Assistants (RNA) are Certified Nursing Assistant specially trained and overseen by the Rehabilitation Staff to deliver restorative treatments such as range of motion and ambulation to maintain a person’s present functional ability.
In some cases, patient’s reach a point in their recovery where they require repetition in order to overcome a plateau. During this period, the therapist will implement a plan in which the RNA will guide the patient in their treatment until at such time, they have reached a point where the therapist can take over to implement a new set of goals and begin working on this new progression. In other cases, patient’s reach a point where there will be no further progression possible. It is at this point, the therapist will develop a maintenance program for the patient that the RNA will implement. In both cases, the RNA will monitor and evaluate each treatment and report to the therapist any changes in a patient’s condition.
Another aspect of Restorative Nursing at Plaza Healthcare is the Restorative Dining Program. Under the direction of the Speech Therapist, those patients requiring the use of assistive devices to eat more independently are initially monitored by the RNA. Other patient’s that may be involved in the Restorative Dining Program are those patients who require cueing or set up to eat more independently.
Facilities/Equipment
The interventions PT, OT, and ST implement are further enhanced by the availability of the most current equipment each therapist can utilize with each patient.
At Plaza Healthcare, the rehabilitation gym is equipped with all the most updated equipment necessary to assist the therapist and patient in their road to recovery such as;
