Plaza Healthcare Earns 2016 Silver National Quality Award

Scottsdale, AZ – Plaza Healthcare has earned the 2016 Silver – Achievement in Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). The award is the second of three distinctions possible through the AHCA/NCAL National Quality Award Program, which recognizes providers across the nation that have demonstrated their commitment to improving quality of care for residents and patients in long term and post-acute care centers and communities.

Based on the core values and criteria of the Baldrige Performance Excellence Program, the AHCA/NCAL National Quality Award Program challenges member providers to achieve performance excellence through three progressive levels—Bronze, Silver, and Gold. At the Silver level, members develop and demonstrate effective approaches that help improve performance and health care outcomes. Plaza Healthcare is the first facility in over 20 years to achieve this award from the State of Arizona.

As a recipient of this year’s award, Plaza Healthcare can now advance in developing approaches that meet the criteria required for the Gold – Excellence in Quality Award .
The awards will be presented to honorees on October 18 during AHCA/NCAL’s 67th Annual Convention & Expo in Nashville, Tennessee.

HCA HONORS EXCELLENCE IN HEALTH CARE
2016 Awards Recognize Innovation and Superior Service in Long Term Care
PHOENIX – Mimosa Springs/Plaza Healthcare is proud to announce that Tammy Evory has been named a 2016 Leadership Professional of the Year by the Arizona Health Care Association (AHCA) , the state’s largest association and advocacy organization for skilled nursing and assisted living communities. The honor was presented in June at AHCA’s annual statewide conference in Scottsdale, Arizona.
The award is one of seven statewide honors recognizing individuals and organizations that go above and beyond to provide exceptional care to Arizona’s elderly and disabled residents.
Tammy Evory serves as the Assisted Living Manager at Mimosa Springs. Under her leadership, the staff created new amenities for residents including a spa program and sensory stimulation room and tripled the activity program to include live entertainment weekly, cooking classes, games, and family gatherings. Staff members report that rather than giving orders, Tammy leads in a collaborative way, inspiring them to share her vision and believe in themselves.
“These outstanding professionals have chosen to work in a career that is complex, challenging and also deeply rewarding,” said AHCA Executive Director Kathleen Collins Pagels. “Each of the caregivers and organizations selected for these awards – along with all those who were nominated – represent the best-of-the best of our profession.”

For the second consecutive year, Plaza Healthcare has participated in the Arizona Special Olympics’ Over the Edge event. On December 12, 2015, eleven Plaza Employees rappelled down City Scape, a twenty-seven story glass building in downtown Phoenix.
The facility set a goal in April of 2015 to raise $11,000 and through the help of our community, staff, friends, and family, Plaza was able to raise $11,250. According to the Special Olympic Committee, Plaza finished 4th in the state of Arizona for monies donated. Overall, the Over the Edge event raised more than $210,000 ensuring that over 400 athletes would receive a full year sponsorship.

On August 20, 2015, in front of thousands at the Arizona Health Care Association Awards Banquet located at the Talking Stick Resort in Scottsdale, Norm Doty received the Resident of the Year Award. This award was a surprise to Mr. Doty. His sons Stephen and Tim were also in attendance when Mr. Doty received his award.
Gayle Borter, the Director of Activities submitted the Resident of the Year nomination. According to Gayle, “Norm’s calm and caring nature lends stability and a welcoming atmosphere in our activity program. He meets and greets new residents and encourages involvement in the activity program. He greets and welcomes residents, staff and vendors. He is known to greet our older residents with “Hello young lady, how about a movie tonight?” He watches over his neighbors during Bingo ensure confident wins.”
Mr. Doty grew up on a dairy farm in Prattsburgh, NY. As an adult he drove truck for 38 years. He was active with his family and his faith. He has been a resident at Plaza Healthcare since June of 2013.
Organized in 1953, the Arizona Health Care Association has grown into a strong and viable group of long term care providers dedicated to enhancing the quality of life for Arizona’s elderly and disabled in residential settings. Today, membership encompasses the majority of licensed, operating nursing care facilities in the state, who together provide continuous nursing care and assisted living services to over 15,000 residents. AHCA was founded to promote standards and ethics in the profession of long term health care, and to ensure quality care for nursing facility and assisted living residents. AHCA is a non-profit organization supported by membership dues, educational program fees and other services.
Plaza Healthcare has been recognized with a 2015 Bronze - Commitment to Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). Plaza is one of two skilled nursing facilities to receive this award in the State of Arizona. The award is the first of three distinctions possible through the AHCA/NCAL National Quality Award Program, which honors long term and post-acute care centers that have demonstrated their commitment to improving quality care for seniors and individuals with disabilities.
“It’s gratifying to see these providers recognized for their commitment to improving quality care,” said AHCA/NCAL National Quality Award Board of Overseers Chair Christine Boldt. “At this level, Bronze recipients are positioned to achieve even better health and organizational outcomes. I applaud and encourage this year’s recipients to continue their quality journeys.”
Implemented by AHCA/NCAL in 1996, the National Quality Award Program is centered on the core values and criteria of the Baldrige Performance Excellence Program. The program assists providers of long term and post-acute care services in achieving their performance excellence goals.
The program has three levels: Bronze, Silver, and Gold. Centers begin the quality improvement process at the Bronze level, where they develop an organizational profile with fundamental performance elements such as vision and mission statements and an assessment of customers’ expectations. Bronze applicants must also demonstrate their ability to implement a performance improvement system. Trained Examiners review each Bronze application to determine if the center has met the demands of the criteria. Centers may now move forward in developing approaches and achieving performance levels that meet the criteria required for the Silver - Achievement in Quality Award. The recipient centers will be honored during the AHCA/NCAL’s 66th Annual Convention and Exposition, October 4-7, 2015, in San Antonio, Texas.
Plaza Healthcare plans to submit their application for the Silver Award in 2016.
